Blue Cross and Blue Shield provides many insurance options and plans throughout the United States. Depending on your state, Blue Cross & Blue Shield provides Health Maintenance Organization (HMO) plans, Preferred Provider Organization (PPO) plans, combined PPO and Health Savings Account (HSA) plans, and Indemnity plans (non-network coverage). These plans (with regional changes) may be offered on either a group basis to employers for their employees or on an individual or family basis or, in some regions, as both group and individual plans.
